I don’t know about mental illnesses, but Basemental Drugs lets you give your sim ADHD. It’s actually really annoying because your sim gets hyperfocused and you literally can’t make them do anything.

I’m the same. It’s good exposure therapy though. There are also plenty of nice people that will respond to your comments.

Are there any voice assistant solutions that run locally? Don’t really care about conversational stuff. Just the basic ‘lights on’ type commands.
